The first year of grant funding in Oklahoma supports initiatives focused on improving individual and community health services and education

WEST DES MOINES, Ia. – Telligen Community Initiative (TCI), the private charitable foundation of Telligen, Inc., today announced it will provide a $25,000 grant to Indian Health Care Resource Center of Tulsa Inc. (IHCRC) to help fund their Medical Home Model project. The project focuses on providing medical case management for urban American Indians experiencing a biopsychological crisis.

The ultimate goal of IHCRC’s medical case management program is to connect patients to programs that meet basic human needs, to enhance human well-being and to reduce health disparities. IHCRC reaches out to Tulsa’s large urban intertribal population to engage Native American people in these programs.

"The best part of what I get to do is be there for folks in crisis. I often ask patients out of everything they are going through, what one thing would help?" said Deni Fholer, IHCRC Medical Case Manager. “The grant funds allocated to this project will help us resolve basic human needs that often interfere with the health and wellness for patients.”

The IHCRC Medical Case Management project is one of the first selected for a grant award within the state by Telligen Community Initiative. The Oklahoma grant award cycle represent the initial funding awards is what is likely to be near $1.5 million in grant support during 2015. TCI also provides funding support to health-focused community projects in Iowa and Illinois.

“The Medical Home Model will provide a range of health and wellness services tailored towards the medical needs of Tulsa’s Native Americans,” said Matt McGarvey, Executive Director of Telligen Community Initiative. “The need is profound and our contribution will positively impact their ability to serve nearly 725 patients annually.”
